Entry forms must be received no later than Saturday May 13th
We must have entry forms by that date so that we can get a count for food & prizes. If you are having trouble downloading the form please contact Dennis at the telephone number listed below.
The tournament will be held at Lake Last Cast on 1300 N. White Rd in Boonville, IN. Click on the map below to open an interactive map of the location where you can enter a starting address for directions.
